Innovative care delivery program, rolled out across multiple Adventist Health locations, is recognized as an outstanding collaboration between emergency and hospital medicine teams.
EMERYVILLE, CA (September 28, 2021)—At the annual Vituity Partnership meeting, clinical leaders from the emergency medicine (EM) and hospital medicine (HM) practices across Adventist Health hospitals in California and Oregon were recognized for the successful rollout of a transformative hospital-at-home program.
With a hospital-at-home program, certain patients with an eligible diagnosis who normally would have been admitted to the hospital are able to receive hospital-level care in the comfort of their own home, with telehealth visits from hospitalist medicine physicians and daily in-person visits from nurses and other healthcare workers. Eligible diagnoses for hospital at home include congestive heart failure, COPD, and COVID-19 symptoms not requiring ICU care.
The Adventist Health Hospital@Home program is transforming how care is delivered and is testimony to how a health system and Vituity can partner on a project that is win-win-win for patients, clinicians, and a health system. Key to the success of this program is the collaboration between the EM and HM teams to meet a strategic goal and form strong partnerships in an entirely new way of caring for patients.

Vituity clinicians volunteered to lead the implementation effort for Hospital@Home at Adventist Health as the first coronavirus surge hit the West Coast. The team recognized today led the way across an entire health system to demonstrate how patients could be admitted at home instead of into the hospital. This is a revolutionary concept that was a critical factor in maintaining capacity to care for patients during the height of COVID-19 surges. Beyond the pandemic, they’ve proven that this model of hospitalization at home not only works and is safe and effective but is also something that patients and clinicians prefer.
Adventist Health’s 150-bed Hospital@Home program serves heart failure, COPD, and COVID-19 patients who require an inpatient (but not critical) level of care. Hospitalists round on patients via telehealth, while mobile nursing and ancillary teams provide in-home care, including infusions, PT, X-rays, ultrasounds, oxygen, and medications.
In addition to satisfying patients while increasing inpatient capacity, Adventist Health’s program delivers strong outcomes. For the appropriate diagnoses, such as cellulitis, COVID-19, pneumonia, pyelonephritis, etc., Hospital@Home has been proven to improve outcomes, increase patient experience, and reduce readmission. Adventist Health has now admitted more than 500 people to its Hospital@Home program.
